There appears to be a website out there from a company www.vitalrecordsonline.com. This particular company isn’t actually a scam, but they are taking advantage of residents. The site says “for the low fee of $65 they will send the inquirer a Vital Application along with instructions and a prepaid envelope made out to the Iowa Department of Public Health.” They tell the inquirer that they just have to complete the application, have the signature notarized, include a copy of their driver’s license and another $20 check make payable to the Iowa Department of Public Health and mail it. ...
